I did these on myself last week and a friend asked for a how to so here it is after the jump.
You will need white polish, a black nail at polish, a top coat , sheet of paper, and a tooth pick.
Start off by painting your whole nail white

it has this kind of wand inside to make your lines

Do the starting strip which will be the space for his eyes

Next make the lines of his bandages two by two

now for the eyes put some white on the sheet of paper and pick it up with the tooth pick and just dot on your eyes
